Practice HI Subscription Terms

These Practice HI Subscription Terms ("Terms") govern the purchase and use of Practice HI, a subscription-based software platform and related services offered by Avidity Health Care Solutions, Inc. ("Avidity," "Practice HI," "we," "our," or "us").

By purchasing, subscribing to, accessing, or using Practice HI, you ("Customer," "you," or "your") agree to be bound by these Terms. If you do not agree, do not use the Service.

Description of Service

Practice HI is a subscription-based healthcare operations and workflow platform that may include, depending on your selected plan and add-ons:

Clinical workflow and operational tools
Patient engagement and communication tools
Scheduling and intake functionality
Documentation support tools
Billing-related workflows and administrative support tools
Optional add-on products and services

Features, functionality, and service levels may vary based on the subscription plan and purchased add-ons.

Subscription Plans and Billing

Practice HI is sold on a recurring subscription basis. Subscription fees are billed in advance on a monthly or annual basis, depending on the plan selected at checkout or otherwise agreed in writing.

All pricing is stated in U.S. dollars unless otherwise specified.

Payment Authorization

By purchasing a subscription, you authorize Avidity Health Care Solutions, Inc. and its payment processor to charge your selected payment method for recurring subscription fees, applicable taxes, and any selected add-ons until canceled in accordance with these Terms.

You are responsible for maintaining a valid payment method and accurate billing information on file.

Automatic Renewal

Your subscription automatically renews at the end of each billing cycle unless canceled in accordance with these Terms.

Cancellation Policy

Subscriptions may be canceled by providing 30-Day Advance Written Notice prior to your next billing date.

Cancellation requests must be submitted in writing to:

If timely notice is provided, your subscription will not renew for the following billing cycle.

If cancellation notice is received after a billing cycle has already renewed, cancellation will take effect at the end of the then-current billing period.

Unless otherwise agreed in writing, cancellation of your subscription will terminate future recurring billing but will not reverse charges already incurred.

Refund Policy

All fees are non-refundable unless otherwise expressly stated in writing. No refunds or credits will be issued for partial billing periods or unused services.

Account Responsibility

You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account.

You agree to provide accurate and complete information during signup and to keep your account information up to date.

Data and Compliance

You are responsible for ensuring that your use of Practice HI complies with all applicable laws and regulations, including HIPAA and other healthcare privacy laws where applicable.

Practice HI implements reasonable administrative, technical, and physical safeguards to protect data but does not guarantee uninterrupted or error-free service.

No Guarantee of Outcomes

Practice HI is intended to support healthcare operations, workflow management, and administrative processes. We do not guarantee any specific clinical, financial, reimbursement, operational, or business outcome from use of the Service.

Limitation of Liability

To the fullest extent permitted by law, Avidity shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of the Service.

Our total liability for any claim related to the Service shall not exceed the total amount paid by you in the three (3) months preceding the event giving rise to the claim.

Changes to Terms

We may update these Terms from time to time. Continued use of the Service after changes are posted constitutes acceptance of the updated Terms.
